Procedure for Change in Name – in official gazette –
You will need to fill up a form which is available free of cost at the Government Press of your city.
You need to fill in the details as required – like your old name, new name, old signature, new signature and address.
No documents are required to be submitted along with the form.
Charges:
Charges for change of name in the Maharashtra Government Gazette:
- Rs 120 per insertion (for two copies of the Gazette)
- For backward class applicants: Rs 60
- Charges for extra copy of the Gazette: Rs 12 per copy
Cash is accepted at the Government Press though you have option of paying through Cheque/DD.
Backward class applicants are required to submit a Xerox copy of caste certificate of old name as issued by the Collector of the District concerned.
Once the form is duly submitted, it normally takes a month for publication of advertisement in the Maharashtra Government Gazette. The Gazette copy is sent to the address given on the form.

I have given the address of Pune city for the form. For other places, you will need to visit the Government photozinco press of your city. If havent come across online form for the same.

Once u submit ur application, it will be published in the next gazatte publication. So it takes max. 30-35 days for the name change to appear.

You will need to make an application in the state where you were born. I am not sure about the procedure in Uttar Pradesh. You can visit the Government Press of your city and find out about it.

My school certificates, my engineering certificates have different spellings so i want to change my name spellings in them. Also my original name is quite long which i want to make short. I know i have to do a official gazette.
My concern is that will this create problems for my abroad travel as i will have different name on my certificate.

You can submit to the consulate a copy of the official gazette alongwith an affidavit reflecting the change in the name.

You are not required to appear before any authority. So your old and new signature on the form is the only requisite you need to comply with. It can be done by someone on your behalf provided you have signed the form.

You can unofficially keep your old name but for official purpose use your changed name only. By using two names, you can create lot of confusion for others and yourself as well.

I do not have any surname in all my official records. My official name is Ashish Kumar. However, my surname is ‘Chaudhry’ which is not in my records. My son was born two years back and his birth certificate contains only is first name “Mukul”. In the space for father’s name in the birth certificate, it is written as “Ashish Kumar”. I want to add the surname to my son’s name to appear as Mukul Chaudhry. Is it possible through publication in official gazette and I hope there won’t be any problem when I apply for my son’s passport?

You can change the name of your son to Mukul Ashishkumar Chaudhry by publication in official gazette.
However, passport authorities may see the difference in the name as a discrepancy which you may be able to sort out with the help of an affidavit (for exact remedy, you will need to visit the passport office and find out the details).
Another option would be to change your name before changing your son’s through publication but then you will require to change your name in many records.
Hi Akshay, I am from Mumbai. Can you tell me where in mumbai do I get this form. Because i face lot of trouble in applying for anything. My Birth Certificate name is Joshua Vineeth Vincent. But I have to change from that to Joshua Vincent, where Joshua is my name and Vincent is my dad’s name. As we donot use surname because an Anglo Indian do use it.

What is the procedure for changing the name on Birth Certificate?
I am an Indian national working in San Diego, CA, US on H1B and my green card is being processed by my employer. My son’s passport is being expired on July 10th, 2009. I wanted his passport to be renewed. (he is 9 year old)
I am thinking of changing my son’s name’s spelling. In that case I guess, it would be considered as name change.
I have couple of queries regarding this issue.

The form is available in government press.
Once the change in name is reflected in the official gazette, old birth certificate can be corrected by providing the department with a copy of the gazette, affidavit and an application. If that is not possible, you will have to provide a copy of the gazette with the old birth certificate.

There is no need for change in your wife’s name through official gazette.
You can simply attach the details of your marriage registration and get her name changed on relevant documents.
you can use this pdf form available online but u’ll have to submit the form in person only.

The procedure can be done in Jalgaon itself. However, you will need to obtain the necessary form. I am not aware if the same is available in Jalgaon or whether you will require to obtain the form from Nagpur / Pune or other place.
Once you fill up the form and pay requisite fees and send it to the government press, procedure will be complete.

Your dad can visit and get the form and send it to you. You can fill it up and send it back.
For changing the name in passport, I believe you will have to apply for it in India.

I am highly impressed with your quick reply, can u suggest me for this .. here in Dubai Indian consulate requires me to give an name change add in Mumbai and Dubai and get both the paper cut outs to consulate and then fill form to get the name changed in PP which is basically mentioned in the “Observations page” of PP, i was wondering that Govt gazzette will help me as it is almost like personally giving an add in newspaper give me ur comments, also lastly advise me if my dad has to travel all the way from Borivali to Charni road, any other way to get the form., i will wait for yet another quickie from you

Since their requirement is to advt in the newspapers, you should advertise. Free Press Journal and Navshakti are newspapers which are reasonable. Gazette is a govt. publication and is different from newspaper.
I am not aware if the form is available at any other place in Mumbai.

It cannot happen in 2 days. You will have to submit the form and it will be published in the next gazette.

I have got the affidavit for my name change and published this in 2 newspapers.
I have to publish this information in Govt. gazette but unable to find any office for the same. I contacted few lawyers for the same but they also do not have any information regarding this.
I live in Meerut (Uttar pradesh).
Do you have any information on how to get the govt. gazette name change form in meerut/UP or any online form link for the same. I am searching from past 10 days but unable to get any info for the same.

You can state the real reason for change in name. I am not sure about the procedure followed in Tamil Nadu. However, after you submit your form with DD, it is checked and then published in the Government Gazette. Once published, you can start using your new name. It normally takes a maximum of 30 days in Maharashtra.

I am staying in bihar. I have changed my name from VIVEK TO VIVEK KUMAR by an affidavit and publishing it in two local news paper. This is the standard procedure in my state for name change. But when i applied for name change in Director identification number and PAN card they are insisting on gazette notification. However my state doesn’t notify it in gazette . My Question is that can i get it published in other state gazette like maharashtra,new delhi etc.If so what is the procedure.

Since you are residing in Bihar and assuming that you are carrying out business in the same state, question for publishing the name change in gazette publication of other state does not arise. I am not aware of the procedure followed in Bihar.
However, considering your problem, I feel you are right in getting it published in another state and then producing it for name change in DIN and PAN card.

For obtaining a ration card in new name and changing name in passport, you will require to follow the name change procedure.
In Mumbai, AFAIK you can obtain the necessary form from Government Press on Charni Road.
